The Photographic Release Form is
required to be filled out by the model/entertainer who is
submitting him/herself for publication in Midwest Nights
Entertainment Magazine. The purpose of this form is to release
the rights to your image for this photographic session to
Midwest Nights in order for us to be able to publish that person's
image/likeness. Without a completed form on file, publication
will not be allowed.
The other form would be the photographic
release form. This form would be required in the event that a
professional or amateur photographer or copyright holder (in
the even that the original photographer released their rights
to the subject or another public
or private entity) other than the Midwest Nights staff
photographers
shot the photograph for publication. Again, without a release,
the photograph(s) in question cannot be published.
Just open the file below, print it out,
complete the form and deliver it in person during your
photographic session or mail it to Midwest Nights Magazine to the
address on the Contact page of this web site. If mailing, a
photocopy of a valid driver's license, state identification
card or copy of your birth certificate may be required at the
time of your submission for the purposes of age verification.
